The Heart to Heart program is a 5 session education and support program of the Heart and Stroke Foundation of Canada that assists cardiac patients and their families who have, or are at high risk for cardiovascular disease.
Education takes part in a small group setting. The group meets on five occasions for 1.5 - 2 hours at at time.
Each week features a different guest speaker. Information is presented by Nurses specializing in cardiac care, Pharmacists, Dieticians, and Physical Therapists.

Cardiac Procedures, Treatments, and Medications
Eat Heart Smart
Stress and Stress Management
Due to the small nature of the groups, there will be plenty of opportunity to ask questions during the sessions.
Spouses, family members, friends, or other support persons are welcome to attend as well.
The program is free of charge.
Heart to Heart Programs
Heart to Heart Programs are held in the Yorkton, Melville, Preeceville, Canora, Foam Lake, Kamsack and Esterhazy areas throughout the year.
